**Note - The Family Registration Form only needs to be completed one time per family.

Submit ONE CLASS REQUEST FORM PER STUDENT (maximum 3 classes per submission.) If more than 3 classes are requested, simply complete and submit an additional form for that child.

Please understand that this is only a REQUEST to participate in a class; it is NOT GUARANTEED unless you proceed to STEP 2 and pay to hold your spot. Classes and workshops are filled on a first come first served basis.

** Important Note - If you do not see the class on the drop-down list, it has already exceeded class enrollment limits and has been closed. Please select a different option!
